An Upper East Side man died Friday after falling 12 stories while trying to fix a window in his apartment — as his wife watched in horror, authorities said.

Michael Courtian, 55, plummeted from his E. 87th St. building near Third Ave. about 9:30 a.m., cops said. A wrench was found on the ground next to him.

Courtian was repairing a screen when he lost his balance and fell out the open window, sources with knowledge of the case said.

Witnesses said Courtian’s wife was inside the apartment — just a few feet away from him — when he fell.
